Find your way Navigation program makes it easier to get the care you need When you’re dealing with a medical issue, wouldn’t it be great if there were someone to explain exactly how everything works? At Stephanie Tubbs Jones Health Center, there is. A Patient Navigation Center can help you receive the range of healthcare and social services you need to live a healthy life. The goal is to eliminate any obstacles our patients may face in accessing the care they need for themselves and their families. A team of navigators provides individual guidance and assistance, linking you with the appropriate services either at the center, within the health system, or out in the community.
