A certiﬁed nurse-midwife (CNM) combines the disciplines of nursing and midwifery to support families during pregnancy and delivery. He or she is a registered nurse who has graduated from a nurse-midwifery education program accredited by the American College of NurseMidwives (ACNM) and passed their national certiﬁcation exam. Women who are basically in good health and experiencing a healthy pregnancy are perfect candidates for care and delivery by a certiﬁed nurse-midwife. It’s an excellent option for those who want more time, attention and support during their pregnancy and labor/delivery— we offer that “special touch.” Our clients also appreciate that they are active partners in decision-making. It’s very empowering to a woman to have her say-so during this time, whether it’s about pain management or delivery position. If Mom wants grandparents and siblings there, or if Dad wants to help deliver the baby, we generally can make that happen. The bottom line is always safety—for mom and baby. If a complication arises, we work closely with OB/GYNs to assure a healthy outcome. And our new nurse-midwifery program at St. Anthony Central Hospital assures the technological excellence is there as well. When we read a John C. Roth, M.D. screening mammogram, our entire focus is to look for indications of breast cancer. Screening mammography is one of the few imaging tests used speciﬁcally to search for cancer in healthy, asymptomatic women. In fact, annual mammography for women age 40 and older is the single most powerful tool we have to ﬁnd breast cancer in its earliest stages when it is most treatable. On mammograms, breast cancer most commonly appears as an irregular mass or a grouping of tiny calciﬁcations. These clusters most often are associated with the very earliest changes of breast cancer—before a mass can be seen on a mammogram or felt as a lump by the woman or her doctor. About one out of 10 women who have a screening mammogram are called back for a diagnostic mammogram to evaluate a ﬁnding. The diagnostic mammogram is a focused set of images to determine whether there is a problem that needs further evaluation, such as a cluster of micro-calciﬁcations or a mass. Diagnostic mammograms often are combined with breast ultrasound, another useful technique for studying breast masses. In addition, breast MRI is increasingly being used to evaluate women when both mammography and breast ultrasound are inconclusive. —John C.
