Improve health outcomes The Congestive Heart Failure Self-Care College at NMMC-Tupelo improves quality of life for patients and reduces the 30-day readmission rate for many patients with this diagnosis. The program’s goals are to reduce symptoms, such as shortness of breath, fluid retention, fatigue and weakness, and to educate patients on how to better manage their condition. This approach reflects coordinated plans of care between the hospital setting, home health care, clinics and Nurse Link with active involvement by the patient. NMHS’ annual Safety Summit included the presentation of the first Impact Award, which was presented to the Door-to-Balloon Project, a multidisciplinary team developed by the NMMC Heart Institute in 2007. The program ensures rapid transport from the emergency setting to the cath lab for an acute heart attack. This year marked the rollout of the Comprehensive Unit-Based Safety Program at NMMC-Hamilton as well as NMMC-Tupelo’s Critical Care Unit and Surgery. Work also continues on concentrated patient safety initiatives specific to fall prevention, pressure ulcers and central line infections. The Critical Care Unit, which has a high patient volume, marked 300 days without a pressure ulcer.
