Community HealtH in Health, Community matters How one group is quietly making a difference For example, the North Brevard Coalition represents 104 agencies, and United Way represents or funds 43 agencies. All told, CHP receives input from approximately 300 organizations throughout the county. CHP’s true strength is the innovative approach it takes to community health, and the high level of involvement and commitment of its diverse members. Needs that are being addressed include service gaps and limitations, and agencies that provide similar services. Call for Help One major milestone for CHP is the full-time Brevard 211 health navigator, which PMC placed in the 211 office. A trained social worker, the 211 navigator provides assistance so Brevard County residents can access social programs that can help improve their health; find transportation to medical appointments; find resources for prescriptions; and search for access to health insurance and mental health programs. The navigator also provides additional educational resources to promote healthy choices for smoking cessation and weight management, health risk management and chronic disease management. “We are excited about our involvement with the Community Health Partnership,” says 211 Brevard Inc. Executive Director Libby Donoghue. “The navigator’s services extend the support, assessment and referral offered by 211 specialists, and provide enhanced assistance to individuals whose social needs impact their health.” T online he Community Health Partnership (CHP) is a diverse group that works together behind the scenes to promote and improve the health and well-being of our community. Parrish Medical Center (PMC) initiated the CHP in 2010 to help community members get the healthcare they need and to prepare for future healthcare changes. CHP comprises more than 40 members from 22 groups, including health and human services, municipalities, schools, civic groups, nongovernmental social service agencies, businesses, local and state associations, churches, state departments, law enforcement, fire departments and libraries. But that’s not the limit of its reach into the community. The member organizations were chosen because they represent many other agencies throughout Brevard County. SoCial Support important, too Because of the high number of layoffs in our county in the last few years, another CHP initiative is the collaboration to make food stamp sign-up easily available at local public facilities. This makes them more accessible for community residents. But CHP members know that’s not enough. CHP has also collaborated with Brevard Workforce to help these same people with work-skills training, résumé writing and interview skills assistance, and made job listings available so they won’t need to stay on food stamps. The Community Health Partnership works together for the good health, and well-being, of all Brevard County residents.
