This adhesive is designed to sustain delivery of the active component over several days following topical application.18 As with any type of drug formulation, there are advantages and disadvantages to its use. The most common disadvantage reported with the use of transdermal drug-delivery systems is the possible development of local skin irritation. Erythema, itching, and local edema can result from the drug, the adhesive, or other excipients in the patch formulation.19 In most cases, site rotation can minimize irritation.19 However, several advantages have been noted with the use of transdermal delivery systems over more traditional dosage forms. In general, the steady per meation of drug across the skin allows for more consistent serum drug levels.19 The lack of peaks in plasma concentrations can reduce the risk of AEs. If toxicity develops from a transdermal drug, the effects can be limited by removal of the patch. Transdermal delivery systems avoid degradation in the GI tract. Convenience is another advantage, particularly when patches are applied once every several days. Simplifying administration helps patients become more adherent to their medication regimens. Transdermal drug delivery is also useful in patients who cannot tolerate oral dosage forms (e.g., patients with nausea or vomiting) and is less invasive than IV or subcutaneous administration, which can be quite painful.19 8
