For Kirsten Platte, executive director of the Community Health Access Network (CHAN), the recipient of the 2008 Davies Community Health Organization Award, the application process gave the entire Congratulations organization a clear view of the “big picture.” “It's been extremely valuable to us to be able to gather history on our EHR implementations and accomplishments into one document that gives us visible weight. This document has validated our progress for staff, and demonstrated to them just how well we're doing and how far we have come as an organization,” Ms. Platte explained. CHAN is a New Hampshire-based Health Center Controlled Network (HCCN), whose community health center members serve more than 75,000 patients. CHAN began EHR implementation nearly 10 years ago, with the last of its 10 sites going live in 2005. With such a mature EHR, the network has been able to track and trend vast amounts of data to improve patient care. “It's one thing to collect the data, but it's another thing to actually utilize the data to improve patient care,” she explains. “Our system has allowed us to excel in that area.
