You and your fellow members deserve recognition! Submitting your chapter’s material for entry in CMSA’s CEIA awards competition will show the rising stars in your chapter how much they are appreciated. CHAPTER EXCELLENCE AND INNOVATION AWARD CEIA CATEGORIES: • Best Use of Technology • Conference Planning • Educational Programming • Membership Development • Print and Publications • Public Policy and Advocacy How to Submit Your Entry/Entries: 1) Fill out the entry forms as completely as possible. 2) Photos are welcome, but optional. 3) Concentrate on the goal, its impact, the outcome and significance of the success. 4) Include concrete information on how the activity improved the lives of those involved or the community at large. 5) Please note that in order to streamline the entry process, all entries are to be submitted via email to INDIVIDUAL MEMBER AWARDS CMOY & AOSE – Awards for the Extraordinary CM Do you know a case manager who stands out in his/her area of expertise? We know the answer must be “YES!” If that one-of-a-kind individual merits the Case Manager of the Year (CMOY) or Award of Service Excellence (AOSE), please make time to nominate that special professional. It is as easy as completing the appropriate form and emailing it to awards@ Review each award description and consider your nominee(s). Then complete the CMOY or AOSE nomination forms and submit them to CMSA National by February 14, 2012. PRACTICE IMPROVEMENT AND RESEARCH AWARDS CMSA’s Case Management Practice Improvement Award and Case Management Research Award recognize an individual, group, or organization that uses findings from a research or quality/performance improvement (QI/PI) project for innovation in the advancement of case management practice and/or improved client outcomes. The awards are open to any individual, organization or group interested in recognition for their work in the areas of research or quality/ performance improvement (QI/PI) in the advancement of case management practice and or improved client outcomes. ELIGIBILITY: Entries must represent projects completed by the award submission deadline. CHOOSING AWARD CATEGORIES: Entrants may nominate their project in one of the following categories based on the criteria for each award. • Practice Improvement Award – a quality improvement/performance improvement • Research Award – a completed research study (with minimum of 6 months of data) Award submission deadline is April 21, 2012.
