Continued from page 7 communication and collaboration with the interprofessional team can result in fewer medical errors, elimination of unnecessary or duplicate tests, improved medication adherence, and ultimately lower rates of preventable hospital readmissions. HIT  MAKING IT A SURE BET In 2012 and beyond, case managers must be knowledgeable about advancements in HIT and the implications for our practice. In order to be effective advocates for our patients, and for the profession, we need to: • Embrace new health information technology solutions as state-ofthe-art tools to effectively communicate across the continuum of care, promote collaborative practice, and improve patient care and outcomes. • Demonstrate a competent understanding of how emerging technology can be used to promote effective care coordination within interprofessional models of practice and care settings. • Advocate for the appropriate use of technological tools and applications that enable ethical and informed decision making by patients and families. • Always remain vigilant in protecting data privacy and security to ensure patient privacy with input, storage, transmission, or retrieval of electronic health information. • Keep an open mind to new, creative, and resourceful ways of using technology to empower and engage patients and promote patient-centered care throughout the care continuum. The use of information technology, particularly EHRs, is clearly vital to implementing a transformed health care system to improve the access, affordability, quality, and safety of health care in our country. Case managers have an obligation to advocate for better health outcomes for patients, practitioners, communities, and the nation – and to cheer on the new technologies that will get us there.
