Sharp’s ACP Program: Targeted Education for the 3 Stages of Life

espite our best efforts, many Americans still do not understand the value of advance care planning (ACP) discussions and do not receive adequate assistance in completing advance directives. Often, they are confused about the differences between living wills, powers of attorney, and the decision-making role and authority of a healthcare agent. Selection
of a healthcare agent is also often done without sufficient reflection on who could best perform this role, and often occurs without discussion with that individual. In May 2010, Sharp HospiceCare, based in San Diego, CA, launched an innovative ACP Consultation Program to help fill these important education gaps. It also strives to address the broader issue noted
by Drs. Rebecca Sudore and Terri Fried in their article, “Redefining the ‘Planning’ in Advance Care Planning.”1 They write: “The main objective of advance care planning, rather than advance treatment decisions, should be to prepare patients and surrogates to work with their clinicians to make the best possible in-the-moment medical decisions.”
