Notary Notes Magazine - DEMO - (Page 19) MOTOR VEHICLE NEWS Section D – Application for Free Issuance Notarization is required only if the applicant is requesting a replacement registration plate or a free replacement of a previously issued product that was not received within a 90 day period from the date of issuance. In Section D, the applicant and any co-applicant must sign the affidavit in the presence of a notary, stating that the registration materials were never received due to loss in the mail. If the vehicle is owned by a company or corporation, a letter on company letterhead is required to authorize the person who signs in Section D to pick up the replacement registration plate. The agent must keep a copy of this letter on file for three years. Appropriate fees should be included with the MV-44 in the form of a check or money order made payable to “Commonwealth of Pennsylvania.” Form MV-913 Revised for 2008 NASCAR Race Season F orm MV-913 – Application for Victory Junction (NASCAR) Registration Plate (4-08) has been updated for the 2008 race season. The updated form, available on PennDOT’s Web site, is valid through April 2009. The cost of the Victory Junction (NASCAR) plate is $54, of which $34 goes directly to the Victory Junction Gang Camp. The Camp’s mission is to enrich the lives of children with chronic and life threatening illnesses by creating camping experiences that are fun, exciting, physically safe and medically sound. Victory Junction is located in Randleman, N.C. and was founded by NASCAR driver Kyle Petty and his wife Pattie in honor of their son Adam. Adam Petty was killed in May 2000 during practice for a race in New Hampshire. Environmental Protection Publishes Final CARB Certification Regulations he Department of Environmental Protection (DEP) has the final regulations for the Pennsylvania Clean Vehicles (PCV) Program on the DEP Web site. The document is dated April 5, 2008. T The 53-page document, PCV Program Technical Implementation and Operational Guidance, has questions and answers for vehicle dealers, issuing agents, and motorists regarding CARB certification. The document includes PennDOT’s Driver and Vehicle Services Update on CARB certification, as well as a number of examples of compliant Manufacturer’s Certificates of Origin (MCOs) and examples of Vehicle Emission Control Information (VECI) labels. Old Editions of MV-4ST are Obsolete June 1 ffective June 1, 2008, the Pennsylvania Department of Transportation (PennDOT) will no longer accept the May 2000 (5-00) edition of Form MV4ST – Vehicle Sales and Use Tax Return/Application for Registration. E All agents under contract with PennDOT should use the July 2007 (7-07) edition of the MV-4ST to avoid having their paperwork rejected. PennDOT announced in the December 2007 Driver and Vehicle Services Update, Bulletin No. 07-10, that the 5-00 edition of the MV-4ST would be obsolete on June 1, 2008. This date was chosen to allow agents enough time to use up their existing supplies of the form. The 7-07 edition has space to record the applicant’s full middle name as well as his or her driver’s license or photo identification number and date of birth. Currently, this information is not captured in the CARATS system, and the application should not be rejected if the information is not provided. Also, the seller (and co-seller, if any) is not required to sign the MV-4ST. The seller’s signature for the odometer disclosure on the back of the Pennsylvania title is sufficient. Controlled multi-part forms such as the MV-1, MV-4ST and MV-120 must be ordered from PennDOT using Form MV-511B. This form, available on the Web at www.dot3.state.pa.us/pdot forms/mv_forms/mv-511b.pdf, has a revision date of 8-07. PennDOT recommends that agents order no more than a two- to three-month supply of forms. Reminder: Orders that are carried to PennDOT and picked up by a messenger service will not be charged a shipping and handling fee. Agents who wish to bring their orders to the Regulated Client Services Section (Dealer Counter) may drop them off there for processing. However, orders will not be filled by the Dealer Counter as the Dealer Counter is not a forms supply area. Orders that are dropped off at the Dealer Counter must have a check or money order attached for shipping and handling fees. Orders that do not include the shipping and handling fee will not be filled.
