Cardiology PACS & CVIS: Implementing Digital Strategies - November/December 2008 - (Page 12) AT THe HeART of SUCCeSSfUl ClInICAl & IT InTeGRATIon: BeST PRACTICeS In CVIS newest facility opened last November, interventional cardiologists and cardiovascular surgeons identified viewing of cath lab images as a critical need for the remote facility. “CVIS requires communication and consensus. Stakeholders must see the value in transitioning to new workflow,” Myers says. At Scottsdale Healthcare, radiologists reached consensus on PACS fairly quickly, primarily because the system’s 46 radiologists represent a single group practice. In contrast, the system’s 40 cardiologists represent 13 distinct practices. Gaining consensus among such a diverse group can be challenging, says Myers. Scottsdale Healthcare bettered the odds by identifying a pair of physician liaisons to help build consensus among their colleagues and provide physician insight. The project team continues to share Centricity workflow with the liaisons prior to the go-live of any major decision point, seeking input and clarifying direction. Phase two of Scottsdale Healthcare’s transition to It’s important to combine digital cardiology technology included the deployment of six digital cardiac cath labs in 2006 and 2007. user input with ongoing departmental observations, says Myers, because clinical staff tends to take their daily workflow for granted. For example, after department and IS team process reviews, Myers created and revised order placement and workflow to meet the needs of the new digital environment. From vision to practice Visionary leadership is an absolute prerequisite to CVIS success, says Morehouse. Success hinges on the ability of a leadership team to direct all players: physicians, administrators, clinicians and IT staff. At Scottsdale Healthcare, the leadership team assumed broad responsibilities from defining the project vision to developing a timeline and identifying resources and funding. “The team needs to build a three- to five-year vision and plan accordingly with various project components and workflow changes. All components need to correlate,” says Morehouse. For example, as Scottsdale Healthcare constructed its vision, the leadership team realized its solution went beyond IT hardware and also required new cath and ultrasound equipment. As the team honed in on the actual practice of a CVISenabled enterprise, they identified key staff and resources. Myers and Morehouse formed the clinical and IT implementation team. Charged with day-to-day project management, the pair brings clinical and information systems expertise to the project. Scottsdale Healthcare also identified critical new responsibilities in the CVIS enterprise. The PACS imaging informatics professional staff provides support, training and server management. each hospital also identified cardiac nurse data specialists to maintain CVIS clinical database integrity and oversee registry reporting. In addition, a data coordinator assists with compiling CVIS reports used for both internal benchmarking and practice improvements. Finally, CVIS training is a critical, complex and ongoing process. Scottsdale Healthcare started the CVIS training process with Ge Healthcare Centricity DMS and ComboLab application training and provides continued training in CVIS and image management workflow. “This is driven internally by user-defined needs and developed in partnership with Ge,” says Myers. CVIS lessons at a glance As Scottsdale Healthcare continues their Centricity CVIS deployment, the team outlines their path for sustained success: › Start with visionary leadership and build the project from agreed goals. › Identify key cardiac and IT liaisons and collaborate with them to build support for the project. › Map and re-map workflow to optimize the installation. › Make sure to assign new tasks, identify critical infrastructure and provide ongoing training and support. 12 Cardiology PACS & CVIS: Implementing Digital Strategies november/December 2008
