Visions - Outpatient Care - (Page 2) Utilizing a 12-step philosophy, the Outpatient Program is divided into three separate phases. Throughout the first ten weeks, clients and their families attend the Primary Care Phase of the program. During the first phase, treatment focus is on disease education, identification of relapse triggers, and an introduction to the 12-steps. Staff develops an individual treatment plan addressing family, school and individual barriers to recovery. Following Primary Care, clients transition to the Continuing Care Phase of the Outpatient Program for months three through six. The focus shifts to relapse prevention, practical application of learned recovery tools, peer support and service. During the last six months, the Aftercare Planning Phase, clients attend the weekly Peer Support Group and are welcomed to attend community reintegration activities as scheduled. This phase is a step toward lifetime maintenance. Clients consent to random drug testing performed at staff discretion throughout the program. Details 2 outpatient program
