MIPS IN 2018-
KEY CHANGES FOR OPHTHALMIC PRACTICES

rom extended transition
flexibility, to small practice
policies, to incorporating the
Cost category, and to scoring
performance improvement,
CMS has made significant
changes in the second year of
MIPS. ASCRS*ASOA remains the
administrator's source for success in
the program.
EXTENDED TRANSITION TO
AVOID A PENALTY IN 2020
Following ASCRS and medical
community advocacy, CMS is
extending transition flexibility
through the 2018 program year,
allowing physicians to avoid the
5% penalty in 2020 by submitting
minimal data. CMS again set the
MIPS performance threshold low
at 15 points-up from 3 points in
2017-which can be achieved in
several ways. Options for scoring
at least 15 points include, but are
not limited to
* full participation in the Improvement Activities category, such as submitting one
high-weighted activity or two
medium-weighted activities
for small practices, or two
high-weighted activities, four
medium-weighted activities, or
a combination of both;

* the Advancing Care Information
(ACI) category base score and
one quality measure meeting the
measure threshold, or data completeness, but not benchmarks;
* ACI base score and one medium-weighted improvement
activity; or
* six quality measures meeting data
completeness, but not measure benchmarks.
Practices may meet the threshold

In addition, CMS is again setting the exceptional performance
threshold at 70 points. Practices
that score above 70 are guaranteed
a bonus in 2020 from this source
of non-budget neutral funds. This
extended transition flexibility of
allowing physicians to avoid penalties will again mean that those
seeking to participate fully and
achieve a bonus will need to score
at least 70.
CMS has also increased the
low-volume threshold in 2018.
Physicians or groups billing less
than $90,000 in allowed Part B
charges or seeing 200 or fewer Part
B patients will be excluded from

MIPS. While this will not impact
most ophthalmology practices,
those in areas with high participation in Medicare Advantage
might be excluded. Check CMS
site qpp.cms.gov to find out if
you're excluded.
For physicians and groups who
want to participate fully in MIPS
in 2018, the performance period
for the Quality category has been
extended to the full year; however,
the Advancing Care Information
and Improvement Activities performance periods remain 90 days.
HELP FOR SMALL PRACTICES
One of the key changes benefitting
ophthalmic practices in 2018 is
expanded accommodations for
small practices of 15 or fewer Medicare-eligible clinicians. Eligible
clinicians include MDs/DOs, ODs,
CRNAs, PAs, and Nurse Practitioners. Group size may include
clinicians who are not eligible
for MIPS, including low-volume
clinicians, clinicians in their first
year of Medicare enrollment, or
Advanced Alternative Payment
Model (APM) participants.
For 2018 MIPS, small practices
* Receive 5 bonus points added to their final MIPS scores
(which can be used to meet the
