Practice Transition Accreditation Program
The American Nurses Credentialing Center's
Practice Transition Accreditation Program
(PTAP) conceptual model is based
on Patricia Benner's novice-to-expert
framework. The program accredits three
programs: RN resident, RN fellowship,
and APRN fellowship. These programs are
designed to help participants move from
entry-level competence toward proficiency
and expertise.
Accreditation, which can be for a single
or multiple sites, is awarded for up to 4
years. Organizations pay an application
fee (based on the number of sites) and an
annual fee of $3,100. Those that receive
accreditation also must submit an annual
report.
Why invest in PTAP status when hospitals
face financial challenges? " Having
accreditation gives us additional structure
to the evaluation process that we already
do, " says Megan Dorrington, MSN, MBA,
RN, NPD-BC, CPN, nursing professional
development specialist for the nurse residency
program at Akron Children's Hospital
in Akron, Ohio. " It helps us look at the
strengths of our program as well as opportunities
for improvement. " The hospital was
accredited with distinction first in 2018
and again in 2021. Dorrington adds that
PTAP aligns with Magnet® standards of excellence
and connects them with a network
of colleagues focusing on successful staff
transitions.
" Being accredited is a good recruitment
strategy, " says Racquel McCrea, MSN, RN,
NPD-BC, nurse residency program administrator,
department of clinical education
and professional development, for Luminis
Health in Annapolis, Maryland. McCrea
says that accreditation sets Luminis's nurse

the OR. Both nurse residency and nurse
fellowship programs can be accredited
through the Practice Transition Accreditation
Program (PTAP) offered by the

residency program, which has been accredited
since September 2020, apart from
others in the state, so the status is promoted
on the organization's website.
PTAP's conceptual model has five domains
that a program seeking accreditation
has to show they meet criteria for:
➤ Program leadership: oversight of assessing,
planning, implementing, and
evaluating the program in adherence
to ANCC PTAP criteria (eg, description
of how the program director evaluates
the program and determines learning
needs of participants)
➤ Quality outcomes: measure of program
impact related to patients,
clients, residents/fellows, and the
organization or practice setting (eg,
satisfaction of participants and managers,
retention rate)
➤ Organizational enculturation: how
participants are assimilated into the
culture, practices, and values of an
organization or practice setting (eg,
description of how participants are
introduced to professional scope and
standards of practice)
➤ Development and design: Building
infrastructure, processes, and
competency requirements to meet a
program's defined objectives, requirements,
and goals (eg, have to submit
competency assessment tool)
➤ Practice-based learning: Takes place
in the workplace setting under guidance
of preceptors, mentors, and
other experienced healthcare professionals
(eg, promotes evidence-based
practice).
AORN recommends that residency programs
be accredited through PTAP.

Larkin says St. Charles Health System
started its OR paid fellowship program
in May 2021, It consists of 12
weeks didactic instruction with clinical
experience, followed by 8 to 12 weeks
of clinical in different specialty areas.
Larkin recently added a requirement for
fellows to complete an evidence-based
project; topics might include pressure
injury prevention or smoke evacuation.
A medical librarian helps support the
nurses in this work.
Mosaic Life Care in St. Joseph, Missouri,
has a more accelerated timeline,
with the bulk of information coming
in the first 2 to 3 weeks. " We do lots
of hands-on training via videos, simulations,
and didactic education in a
classroom or OR setting, " says Mary Jo
Hutchcraft, MSN, RN, CNOR, nurse education
specialist for surgical services.
" It's intense. "
The nurses then work with a preceptor
circulator for the next 6 months.
Orientees may have the opportunity to
scrub, but more commonly, that experience
comes later in their employment,
if at all. The hospital has 14 ORs and
completes about 35 to 50 cases a day.
Moving from a position where the
nurses felt comfortable with their knowledge
to a new specialty can be daunting.
" It's challenging to go from being an
expert to being a novice again, " Hutchcraft
says. " It can be hard on their confidence. "
Part of the challenge is how
different the OR is from a nursing unit.
Hutchcraft tells nurses they will still use
some of their skills, such as assessment,
as they learn new ones. " I try
to be empathetic and give them the
tools and resources they need to learn
and improve their self-confidence, " she
says. That might include recommending
a YouTube video about the next day's
surgery, sharing articles, holding interactive
discussions, or providing hands-on
opportunities.
Anita Wilson, MSN, RN, CNOR, the
OR nursing professional development
specialist at Akron Children's Hospital
in Akron, Ohio, finds it helpful to set
expectations from the start. " I tell them
that they are going to learn totally new
