Perioperative Survey
Select demographics of nationwide sample of OR nurses

n the March 2022 issue of OR Manager,
the Competency and Credentialing
Institute (CCI) presented and
analyzed the current sources of data
that have historically made up a demographic
representation of the perioperative
nursing workforce. Our article also
spoke to the issue of the lack of coordination
among the groups who have
gathered this existing data.

One thing is clear: Several anecdotal
reports and surveys point to the
increasing difficulty in hiring surgical
technologists, perioperative, and perianesthesia
nurses. Although there are
many stakeholders effected by these
shortages, there is no one agency or
administrative body that takes overall
responsibility to address these scarcities.
There is a compounding factor to
these staffing shortfalls. The lack of
reliable, actionable data hamstrings the
perioperative nursing specialty and puts
the profession in a reactionary mode
when addressing these shortages.
The demographic makeup of the perioperative
nursing workforce is of great
interest to CCI. From a strategic planning
standpoint, knowing the age, experience
levels, race, and ethnicity data
of perioperative nurses would help CCI
to best meet the professional development
needs of the certificants our organization
serves. CCI also believes that
we can be part of the solution to the
complex, multi-faceted issue of perioperative
nursing shortages.
What is the importance of
nursing workforce studies?
The current data on the makeup of the
perioperative nursing workforce is fragmented
and incomplete. Reliable data
is lacking because mostly large, representative
(random) samples have not
been done. Small survey-based samples
targeting subgroups of the specialty
constitute the best current data.
These surveys are done by membership
organizations, trade publications, or certification
bodies.

There are only episodic and uncoordinated
attempts to gather and operationalize
data on the thousands of
perioperative nurses currently practicing.
Thus, effective workforce planning
for the specialty does not exist. In such
a scenario, the staffing shortfalls, competition
for scarce human resources,
and subsequent detrimental effects on
the provision of surgical care are bound
to continue.
The American Association of Colleges
of Nursing (AACN), in a 2019 publication
titled " Fact Sheet: Enhancing Diversity
in the Nursing Workforce, stated:
" Nursing's leaders recognize a strong
connection between a culturally diverse
nursing workforce and the ability to provide
quality, culturally competent patient
care. [...] With projections pointing
to minority populations becoming the
majority by 2043, professional nurses
must demonstrate a sensitivity to and
understanding of a variety of cultures
to provide high quality care across settings. "
In addition, the 2020 US Census
informs us that individuals from ethnic
and racial minority groups account for
nearly 40% of the US population.
In a 2017 Position Statement, AACN
stated that collectively, nursing education
can " address pervasive inequities
in healthcare by ensuring the preparation
of nurses and other healthcare
professionals able to meet the needs
of all individuals in an increasingly diverse
American society. " CCI believes
that credentialing bodies may also have
a role in promoting a diverse nursing
workforce.
If we agree with the statement by
AACN that a diverse workforce that mirrors
the overall American population will
facilitate high quality, culturally competent
care, the question then becomes,
how diverse is the US nursing workforce?
Other
questions emerge if we examine
the AACN statements. For the
perioperative nursing specialty, we
must also ask: How diverse is our workforce
compared to both the total American
nursing workforce and the overall
population for which we care? CCI is
addressing these questions in our research
efforts.
CCI research on demographic
makeup of perioperative nursing
In 2021, CCI, in conjunction with the
American Board of Nursing Specialties
(ABNS) and The Accreditation Board for
Specialty Nursing Certification (ABSNC),
funded research that will provide more
definitive data on the makeup of the
perioperative nursing workforce. The result
was a study conducted by CCI staff
and contractors, titled Investigation
of Perioperative Nurses in the United
States. For this article, this study will be
referred to as the 2021 CCI Study.
The target population of the study
was RNs across the US in surgical services,
including preoperative, intraoperative,
and postoperative. The aim was to
" investigate how individual factors (eg,
certification status and nursing experience)
and organizational factors (ie,
nursing practice environment) influence
perceptions of perioperative competency
among perioperative nurses. " The
study was a quantitative cross-sectional
design with descriptive and comparative
components. Institutional Review
Board approval was obtained through
the Western Institutional Review Board,
which granted this study Exempt Status
determination.
The study was conducted entirely
online to gather participants' self-reported
data via a survey. Participants
were recruited via online newsletter notifications,
e-mail announcements, social
media posts, and notices placed
in industry trade publications.
Industry
partners worked closely with CCI in
disseminating recruitment messaging.
Respondents were incentivized with
drawings for electronic devices to spur
responses. Participation in the drawing
for the incentives was voluntary and not
a condition of participation.
