Effect of union membership on
nursing turnover, job satisfaction
This study, led by Lewis College of
Business, Marshall University, South
Charleston, West Virginia, examines the
relationship between nursing unions
and nurse turnover and job satisfaction.
A total of 43,960 nurses from the
2018 National Sample Survey of Registered
Nurses were included in the analysis.
About 16% were represented by
labor unions. Among the findings:
* Overall turnover rate was 12.8%,
with union nurses less likely to report
turnover than nonunion nurses
(mean 10.9% vs 13.16%)
* Union nurses reported less job satisfaction
than nonunion nurses (mean
3.20 vs 3.28)
* Union nurses were more likely
than nonunion nurses to be male
(12.72% vs 9.46%), minorities
(37.65% vs 25.67%), and employed
in a hospital (70.1% vs 57.9%)
* Union nurses reported fewer hours
of work per week than nonunion
nurses (mean 36.73 vs 37.66)
* Adjusted analysis showed a positive
association between union status
and turnover, but an inverse association
with job satisfaction.
The researchers concluded that
unionized nurses were less likely to report
turnover but were more likely to
have job dissatisfaction.

Motor
vehicle crash risk after
general surgery
Because there is limited data to guide
return to driving recommendations for
patients after surgery, researchers at
the Massachusetts General Hospital,
Boston, performed this nested casecrossover
study to find if the risk of a
motor vehicle crash increases after surgery,
compared to before.
The analysis included 70,722 licensed
drivers who had inpatient or outpatient
general surgery and were discharged.

crashes in the 4 weeks after surgery
was 263 (0.37%), compared to 279
(0.39%) in the 4 weeks before surgery.
Consistent with nonsurgical populations,
specific demographic groups (ie,
non-Hispanic Blacks, Hispanics, and
those aged 18-34 years) were at higher
risk of a crash after surgery, compared
to non-Hispanic Whites and those ranging
in age from 35 to over 70 years.
Crash incidence after surgery was
lower in male drivers than female drivers
and those with a hospital length of
stay of 1 day or more vs less than 1
day. The researchers concluded that the
incidence of motor vehicle crashes over
a 28-day period did not change on average
before compared to after surgery.

ASA,
APSF recommendations for
elective surgery after COVID-19
The American Society of Anesthesiologists
(ASA) and Anesthesia Patient
Safety Foundation (APSF) on June 20
released a joint statement providing updated
recommendations for the timing
of elective surgery and anesthesia after
COVID-19 infection. They include:
* Patients with COVID-19 type symptoms
should be screened and, if appropriate,
tested before an elective
procedure.
* Elective procedures should not occur
within 2 weeks of infection.
* A patient's physicians should conduct
a risk assessment between
2 and 7 weeks after COVID-19 infection
and, if low risk, consider rescheduling
the procedure.
* Delay of surgery after 7 weeks
should be considered if the patient
still has COVID-19 symptoms.
* Facilities, surgeons, and anesthesiologists
should track COVID-19 patient
outcomes after elective surgery.
These recommendations help provide
guidance on balancing the risk of
delaying surgery with the risk of complications,
ASA says.

New
borescope training model
improves SPD knowledge, skills
This pilot study by epidemiologist Cori
L. Ofstead, MSPH, and colleagues finds
that a new borescope training model
improves sterile processing department
(SPD) personnel mastery and retention
of complex skills.
The study involved nine certified SPD
personnel. Training focused on borescope
visual inspection of endoscopes,
pre- and post-training tests, structured
homework, and an online booster session.
None had a passing score before
the training; 100% passed the test following
it. Mean test scores increased
significantly after the workshop (41%
vs 84%), and improvements were maintained
after the booster session.
All trainees identified clinically relevant
visible defects on patient-ready endoscopes
at their institutions, and they
reported higher technical confidence
and satisfaction levels after training.
Further research is needed to determine
if this training model is scalable, the
authors say.
