fforts in 2017 to dismantle the
Affordable Care Act (ACA) were
largely unsuccessful, but a tax
reform bill that includes a provision to
abolish the ACA's individual insurance
mandate seems likely to pass.
As a result, 13 million fewer Americans
will be insured by 2027, and premiums
will go up annually by an average
of 10%, according to the Congressional
Budget Office. Other potential consequences:
Healthier people will drop coverage,
insurers will leave the exchange
market, and the federal deficit will balloon,
industry leaders say. Their outlook

for the coming year includes:
* a continued focus on cost cutting
* more emphasis on value-based care
and contracting
* more outpatient care and use of " virtual
care " (ie, telehealth)
* patient demand for cost and quality
transparency
* more payer-provider partnerships.
This list comes as no surprise, and
it reflects some of the challenges facing
healthcare providers. As one example
of those challenges, a recent
University of Utah survey revealed a
wide range of interpretations for the
meaning of " value. "
Survey respondents (5,031 patients,
687 physicians, and 538 employers) all
agreed that healthcare costs too much,
but they disagreed about what constitutes
value. Almost 60% of employers
stated that cost is a key component
of value, whereas physicians equated
the word with quality measures, and
patients defined it as quality, cost, convenience,
and customer service.
University of Utah chief medical quality
officer Bob Pendleton, MD, noted
that although 76% of physicians said
they consider cost when making treatment
decisions, they often lack accurate
information. And the vast number
of different insurance plans only makes
it harder for physicians and patients
alike to understand what things cost.
We don't have all the answers. But
we do have the wisdom of experienced
healthcare leaders. In this issue, for
example, we drill down into Medicare
payment models (p 20) and provide an
update on quality reporting deadlines
for outpatient procedures (p 29).
Collaboration with experts like
these will help meet our goal for
2018: To continue as your primary
source of news and information. We
