Hermanson, DNP, MBA, RN, NEA-BC,
CASC, administrator at Ambulatory Care
Center in Vineland, New Jersey, showed
how the worst failures of communication
and teamwork can lead to the
unthinkable: wrong-side surgeries and
other critical mistakes. Among other evidence,
she cited Agency for Healthcare
Research & Quality (AHRQ) statistics,
showing 80% of medical errors result
from inadequate handoffs. Worse, only
8% of nursing schools teach how to
hand off in a formal, didactic class.
The good news: Teamwork and communication
are teachable skills, Hermanson
said. Much of the subsequent
presentation consisted of an overview
of TeamSTEPPS, an evidence-based
framework developed by AHRQ for improving
teamwork and communication.
One of many examples of the TeamSTEPPS
tools and strategies Hermanson
covered is the two-challenge rule.
Some nurses are less assertive than
others, but a rule stating it is their responsibility
to assertively voice a safety
concern at least twice can empower
them to do what is necessary. The rule
also requires team members to acknowledge
they have been addressed.
In addition to ensuring the concern is
heard, the rule directs personnel to
take the concern up the chain of command
or otherwise take stronger action
if outcomes are unacceptable.
Complementary to the two-challenge
rule is the notion of " CUS " words-
using the terms " concerned " (C), " uncomfortable "
(U), or " safety issue "
(S)-to ensure a message is heard.
Practice makes perfect
These were not the only examples of
the need for effective communication
and teamwork making its way into the
educational sessions. Presentations
dedicated to nurse bullying, the difference
between implicit and explicit
trust, and the creation of an efficiencyboosting
OR " pit crew " were particularly
on point in this regard.

However, I was there for more than
just education. Communication-visiting
Expo Hall booths, meeting with editorial
contacts, and generally seeking
leads and ideas-was something to
actively practice, not just learn about.
Ditto for teamwork, as I did not attend
the conference alone (we hope you visited
the OR Manager booth!).
Thanks to various networking opportunities
and activities around the
expansive Expo Hall, the case was the
same for other AORN attendees. To
name one example, I would have felt
out-of-place attending the " Mystery Escape
Room " activities, but AORN YouTube
video showcases how a clever
take on this popular activity can help
nurses gain hands-on experience with
navigating surgical emergencies. Like
a traditional escape room, participants
had to work closely together on interpreting
clues that point the way to escaping
the room and " winning " before
the time expires. The difference was
that all clues were related to how to
care for the rapidly deteriorating " patient "

Overall, the underlying focus on
teamwork and communication in the
sessions I attended was encouraging.
As Hermanson pointed out during her
overview of TeamSTEPPS, data from
The Joint Commission ranks ineffective
communication among the top three
causes of sentinel events. It follows
that effective communication should
also be a top priority for all healthcare
leaders. After all, the primary goal for
these personnel-and the core reason
for industry events like AORN and
