Recognize, report, prevent: Building accountability for
workplace violence
nyone doubting the prevalence
of workplace violence (WPV)
in the healthcare setting
need only search online for " injuries
to healthcare workers. " In a sector
rife with occupational hazards-from
slips and falls to sharps injuries and
infections-a significant portion of
the links are sure to mention WPV,
with many even focusing on it.
The epidemic even has its own social
media tag: #EndNurseAbuse. In addition
to anecdotes and commentaries, the
long list of search results will include
plenty of hard numbers confirming the
disturbing reality: WPV is pervasive in
healthcare, impacting not just mental
and physical health but also productivity
and turnover rates.

In one example from 2020, the US
Bureau of Labor Statistics estimated
in the private healthcare and social
assistance industries, the number
of nonfatal occupational injuries and
illnesses resulting from intentional
injury by another person nearly doubled
in an 8-year period, jumping from
8,180 in 2011 to 15,230 in 2018.
More recently, a 2022 survey by
Press Ganey revealed that at least
two nurses are assaulted every hour.
This devastating number translates
to approximately 57 assaults per
day, 1,739 assaults per month, and
5,217 assaults per quarter.
This state of affairs is both deplorable
and unacceptable. We can and
must do a better job of recognizing,
reporting, and preventing WPV.
Recognizing WPV
Recognizing WPV requires establishing
a definition that is universally accepted
and incorporated into workplace policies.
The definition published in The
Joint Commission glossary is, " An act
or threat occurring at the workplace that
can include any of the following: verbal,
nonverbal, written, or physical aggression;
threatening, intimidating, harass8

Forms of workplace violence
The National Institute for Occupational Safety
and Health (NIOSH) defines workplace violence
(WPV) as any type of physical assault,
threatening behavior, or verbal abuse occurring
in the work setting. The organization
groups WPV into four separate categories:
Type I: Criminal intent involves an individual
who has no relationship to the business
or its staff. The crime usually is associated
with violence linked to robbery or trespassing.
Type II: Client-on-worker violence occurs
in the healthcare environment between staff
members and individuals with whom they
have a relationship, including patients, family,
and visitors.

concern involving staff, licensed practitioners,
patients, or visitors. "
The Joint Commission has gone
further than defining WPV. It has also
released accreditation standards to
spread recognition of WPV-and resources
to fight it-throughout every
level of individual organizations. As of
January 2022, revised WPV standards
require all accredited and critical access
hospitals to develop education, training,
and resources for all staff members.
Among other requirements, they
must:
* provide a definition and examples of
WPV
* clearly define roles and responsibilities
for all staff
* offer training in de-escalation, intervention,
and emergency response
* provide a process for reporting WPV.
That last item is particularly critical
because the number of unreported
events is staggering. According to the
American Nurses Association (ANA),
as many as 80% of incidents of WPV
Type III: Worker-on-worker violence is
commonly referred to as lateral violence and
includes behaviors such as bullying, incivility,
humiliation, verbal abuse, and emotional
abuse.
Type IV: Personal relationship violence involves
a relationship between the perpetrator
and the victim that is external to the organization
but occurs within the organization.
Type II is most common. In a 2019 American
Nurses Association survey of 20,000
RNs, 31% reported experiencing aggression
(verbal or nonverbal) from peers; 24% from
authority figures; and 34% from patients or
visitors.

is the first step in accountability
toward creating an environment that
strives to prevent WPV, but recognition
must be followed by a culture that encourages
reporting.
Reporting WPV
Gaps in the data show the extent of
the reporting problem. For example,
2011 estimates from the Department
of Labor peg the number of annual
WPV incidents at 22,250. Yet, according
to The Joint Commission, only
256 incidents of WPV were reported
between 1995 and 2010. According
to ANA, there is no standardized process
or requirement to report WPV.
Legislation to help address this
issue (and WPV more generally) is
moving through Congress. In 2021,
the Workplace Violence Prevention for
Healthcare and Social Service Workers
Act was introduced to direct the
Department of Labor to mandate certain
employers to institute workplace
violence prevention plans-including
reporting procedures. Although the
