Efficient Archwire Preparation for
Surgical Cases

Although orthodontists have time to fine-tune their
work over months of active treatment, our collaborating
orthognathic surgeons must make their
alterations in minutes. We can help them achieve
their treatment goals by anticipating needs that may
come up during the relatively abbreviated time in the
operating room. Unexpected changes in their surgical
approach can necessarily develop, and it is our responsibility
to help provide options for them.
Fixation of the facial bones, multiple segments, surgical
templates, and splints are most often provided
using our fixed appliances. Anyone who has seen the
surgical repair results of traumatic injury using archbars
or transcutaneous plates recognizes the limitations
in the accuracy of skeletal reproximation as well as
postsurgical hygiene and inflammation control. Surgeons
using the intermaxillary fixation technique with
Kobayashi ligature ties on individual teeth or crimped
auxiliary hooks on archwires know that archwires can
be distorted, brackets can come loose, auxiliary hooks
can become displaced, or parts can become detached
during and after the surgical procedure. Surgeons may
even have to use the fixed orthodontic attachments
directly to anchor the intermaxillary stabilizing wires.
These techniques can all cause unwanted changes in
tooth movement and arch coordination during the
postsurgical fixation and healing period.
Surgeons have preferred soldered hooks on archwires.
These are time-consuming and technically difficult
to place. Where a surgeon will need the hooks to be
positioned cannot always be anticipated, so it is important
to provide alternate positions for attachments
before surgery. Soldering the wires is more predictable
and secure than welding. Placement of the surgical
hooks interproximally can distribute the forces used to
position the anatomical elements more evenly.
During the past few years, a Canadian company
(Speed System Orthodontics, Ontario, Canada;
speedsystem.com) has developed hooks (Trillium
Compression Hooks; Figure 1) that can be clamped
