Introduction: A few questions about implant-guided cases on here lately, and it seems like some folks don’t understand what is involved in the surgical appointment. I don’t do a lot of guided implants, especially not for partially edentulous cases, and especially not in the anterior. But here you go. We were threading the needle between a large irregular incisive canal and a tipped-in UR lateral root. So it’s a good case to take a few pictures of what happens in the actual surgery. Note that there are a hundred different ways to do this, and none are necessarily wrong. I’m just showing one way that works for beginners to get an idea. Rarely seen in these parts. Fig. 3: So we take a CT at 0.3mm voxel or so, and an alginate just of the arch to be treated (unless a diagnostic wax-up required). Then we uploaded the scan to a design centre–this one was by 3DDX in Boston–and couriered the model to them as well for surface scanning. You can do the design in-house if you have the software, and scan the model or even the patient yourself with some protocols. Fig. 4: After they design the case, they do a go-to meeting session with you online to review the design before production. Obviously for a simple case like this, it lasts five minutes. Fig. 5: A week later you get back a 3D STL model of your arch, with a STL guide stuck on it. This is a Simplant SurgiGuide, made to be compatible with the NobelGuide Replace Tapered surgical kit. This combination makes it easier to go without a stupid scanning appliance for simple dentulous cases. Fig. 6: A better photo. These are both 3.5mm implants, so a 3.5mm sleeve for each and we will fit a 2mm pilot drill sleeve inside it during surgery.
