This is good. Thanks, Lane! The diagram makes sense. So why would we be placing them
on most posterior mandibular 3-unit bridges but not on maxillary 3-unit bridges? (Maybe you
never said this.) Wouldn't the forces applied be the same? Sorry for all the questions! I guess I
now know what I am going to try the next time I treatment plan a bridge! ■

I have a case in progress where the distal abutment of an old bridge 13-x-15 had debonded at
the #15 abutment but was solid on #13. Removed then bridge reprepped. #13 was a tall retentive
prep. I asked my lab for a keyway on the distal of #13. The bridge I got back from the lab has a
keyway on the mesial of #15 with the female on #15 and the male portion on the pontic. Will
this setup be subject to failure, or should I send it back and have it remade? ■

It depends. I like the least retentive prep to have the stress breaker in it, typically that's the bi. ■

As Lane showed, we do use them on maxillary bridges when there's either a long span or a
pier abutment. Generally not needed on short span maxillary bridges because of the direction of
forces. The exception is when one abutment is significantly weaker than the other. ■

I'm having a little trouble understanding how to "reverse the attachment." I understand the
female has to sit in the mesial abutment, with less retention to sit and direct force down that
less-retentive abutment and minimize mesial lifting. I'm having trouble visualizing how the
reverse attachment would work to hide the keyway without placing the keyway into the mesial
of the pontic. I did a search on DT and found another thread mentioning it, but it seems that the
reverse would result in some tipping forces distal to the mesial abutment, right? ■

That's OK, the trick is to allow some independent movement
between the abutments, so the less-retentive one doesn't work
loose. ■
