Just saw this article from an ADA email snippet. I have seen open contact on implant crowns
before but didn't think it happens half the time! The open contact leads to food impaction and
possibly peri-implantitis. Should we warn all patients that their implant crowns need to be
replaced in a few years? Or should we do Class IIs on the adjacent tooth? Force everyone to wear
a retainer or nightguard?

Teeth tend to drift mesially. Implants don't move. One solution is to use a CAD/CAM
crown (E.max or FCZ), save the file and re-mill when it happens for a nominal fee. ■

In my opinion, this is why it's so important to restore with screw-retained crowns whenever
possible. In addition to the elimination of cement sepsis as a mode of failure, the implant crown
becomes easily retrievable and a pickup impression can be made to send to the lab for the
addition of porcelain to contact area. ■

I have a similar situation coming up for a screw-retained crown on #13. If the crown has
been in the mouth for more than two to three years, does the saliva contamination affect the
adding of the porcelain to close the contacts? I remember reading this a few years ago. ■

The last research article I read said 30 percent of mandibular molar implants will develop
an open mesial contact due to naturally occurring mesial drift. We do almost exclusively screwretained implants and either save the model or give it to the patients to keep. Then, if the contact
opens, we have the model to send to the lab to add/re-bake porcelain. ■

Can you please reference the article? Did they document anterior drift, or just assume anterior drift because they were unable to conceive any other way for this happening? In this study,
21 percent were distal open contacts and 47 percent of all open contacts were on anterior teeth.
How does that happen if mesial drift is the culprit? ■

I think the article was posted a couple weeks ago here on DT by a couple of our oral surgeons.
Don't know if they stated cause, but it's mesial drift for the mesial open contacts and cross-tooth
interferences for the distal open contacts. The implants are (hopefully) going nowhere. There
was some concern about continual growth patterns in adults. ■
