Answers:
The appointment time is one full day (8 hours) with a 2-hour follow-up the next day.
I use a putty from a wax design as a lingual incisor guide.
Most of these cases are SB2 from Cosmedent as a base shade and Trans from Tokuyama
for the facial translucent layer.
Notes:
As far as appointment time goes, it really is not that much longer than an indirect case for
me. (Four hours prep and temp, 1 hour total follow-ups, 3 hours seat and bite refinement
= 8 hours), and I don't have the stress of wondering if the lab will send me back something
that I want-will the midline be right, will I like the color and the shapes, will it block out
the discolored tooth, etc. Not to mention that we were able to preserve all of the patient's
enamel!
I use other matrices (Toffelmyer and Mylar) as well.
SB2 does a good job of blocking out tooth color in thin layers (liquid resin opaquers can
also be applied for dark teeth). ■

Pretty spectacular stuff!
One thing that I immediately notice is the camera settings are consistently different in the
postoperative pictures. Is this on purpose? Further, all of the final smiles look very similar to
each other. They look awesome, but very similar. Is the wax-up a digital one using a common
tooth library?
Is this level of result obtainable by a dentist with good skills, but not the skills of a brilliant
artist? Some of Jason Smithson's work is incredible, but layering multiple layers of composite to
the correct thickness is something that most of us just cannot do. He's got special talent that
most dentists don't, even if they are very good.

Beautiful dentistry! And no enamel destroyed or lost. Only problem is too many dentists
don't feel they can do this kind of dentistry. It certainly is not easy, but you can always learn and
improve over time. Thanks for showing what you can do with composite! ■

You say these are direct composites with no prep, yet you describe a prep and seat in your
protocol-call me confused. And if that is real direct composite, that's pretty insane. Yeah, the
"after" photos all look the same. Carry on. ■
