In my experience, violating biological width will defi nitely trigger infl ammatory response.
Patients with excellent oral health and minimal gingival infl ammation will notice it and will
come back with complaints, and patients who already have gingival infl ammation everywhere
and who don't have good oral hygiene don't care and probably won't even notice that. ■

The critical axis for resistance form is buccal-lingual, so if you need to pull up short on the
distal, I wouldn't be too concerned. Another thing is to do a pothole in the occlusal and bam,
that crown will never come off, even if you have 1 mm wall height on the distal. You can also
save more tooth structure and avoid a deep margin that way. ■

I always explain to my patient what I am doing. They know they may be sore. They know
they may have infl ammation that lingers. They are told that CL can be done now or could be
done later if their body doesn't respond well. As I said, and others above have said, our own
clinical experience has shown a great majority of favorable experiences. Maybe it is semantics
and we are actually doing a partial CL. I use a diamond and remove bone where and when I
need to. I want to see nice sound tooth adjacent to my core. I always use white core because I
want to see that delineated area and fi nish my margin beyond it at least 1-2 mm. This is real-life
dentistry. Not academics.
Will patients say yes to adding $800 to their crown bill and going to a specialist (I don't
do fl ap surgery) and getting cut and sutured? Some will.
Most like my idea that I will send them for that if they
don't heal well, and will try without fi rst, and that the
vast majority heal well without. They play the odds and
I do too. ■

width 2.04 mm number is an
average. By defi nition, 50% of patients will have more
than that and 50% of patients will have less than that.
Gargiulo's paper had ranges from less than 1 mm to over
4 mm. ■

cedure that results in a healthy
periodontium. Crown lengthening also makes impressions
far easier and provides more tooth substrate to
retain the crown. I don't know why so many dentists
don't ever perform this operation. ■
