When you say you're more comfortable with a bar if it's not an ideal situation, is that because
you're more comfortable tying the implants together with a bar similar to a hybrid, as opposed
to having individual implants with Locators in those nonideal situations? ■

You used a radiographing guide and planned in software but then freehanded your guide.
Did you open up the slots on your guide based on anatomy in the scan compared to the cast?
Or measure it on your cast? Seems like the fact you have nice bone/tissue makes being really
exact less crucial. What if you had lower or more pronounced sinuses? I never understood how
clinicians freehanded distal angled implants to avoid the sinuses without a guide. My question
is in reference to both overdentures/fi xed. Thanks. ■

Excellent case and documentation, as always. Thanks for sharing. The midline seems like
it would be an awkward site for an implant. Isn't the incisive canal in the way? How do you
manage that? ■

Nonideal situation means that implants can't be placed at ideal angulations or ideal A-P
spread. When this is the case, a bar can make up for some of these factors. The bar will correct
any angulation errors and splinting implants together and cantilevering mesial and distal can
greatly increase A-P's spread. If planning a bar, the most important factor to plan for is adequate
vertical space.
My radiographic guide was a duplicate of the patient's denture with composite in the central
fossa of each tooth. So for my surgical guide, I knew exactly where to cut the hole relative
to the radiographic guide. Clinicians know how to freehand distal angled implants by using
anatomical references (socket, etc.) or by actually exposing the lateral window slope of the sinus
and using that as a reference. Some reference is always needed, or like you said, it's just a guess-
which doesn't work well. ■
