I started placing O rings and locator attachments to retain lower complete dentures more
than 20 years ago after I referred my patients to have two implants placed in the lower cuspid
sites. I still find that this is the best solution for retention and stability. Balancing four implants
does not provide better stability and retention, in my experience. ■

Hey, DoctorEd. I hate disagreeing with a Dentaltown legend ... but your statement is incorrect. Four implants with excellent A-P spread is absolutely more stable and has better retention
than two implants in the canine positions. Always. I can't speak as to why your " experience "
has not supported this statement, but nonetheless, it's true. It's like saying water is wet. It just is.
Even if we are talking about only two implants in the mandible, it is old dogma that the
canine positions are the " correct " location. The best location is dependent on jaw form (square
vs. tapered vs. ovoid). Sometimes the lateral incisor position is more ideal than the canine.
You wanted to know what was opposing the picture from above. Here it is. I would love for
you to explain to me exactly how (be specific!) this case would be more stable and more retentive
if the back implants were gone and the front implants were in the canine position. ■

Nicely done! Luckily, the patient had enough bone to get the spread on the implants. This
denture is going to be rock-solid. I do cases like these routinely and I try to place 5 to 6 implants
if I can-which is an overkill, I know, but that allows me to immediately load a couple of
implants with locators and gives the patient the function till they get the final prosthesis.
Most of the time, the lower dentures are almost nonfunctional during three or four months
of healing and most patients don't wear them. I do additional implants for my own peace of
mind so I can minimize postop visits and increase patient satisfaction. I have been doing this
for the last 12 months and have done 24 cases where I immediately loaded the implants with
locators, provided patient isn't a smoker and I got torque of more than 45NCms. ■
