Deprogram simply means removing biting muscle
memory. You can simply ask the patient to wear a deprogrammer
every night for two weeks and bring him back
on the day of the CR record with the deprogrammer in
his mouth. You are the one who removes it.
Taking CR bite is easy. Put some leaf gauge between
central incisors, have the patient bite down, protrude,
retrude and squeeze down. Make sure molars are separated.
Question is, how many leaf gauges to put between
incisors. The simple answer is until you achieve the
ideal anterior VDO. For Class I patients, it's around
16-18 mm between CEJ to CEJ of central incisors. Class
II is 14-16 mm.
Once you've got CR bite, remove one leaf gauge at a
time until you fi nd the fi rst point of contact. When the
lab mounts your model in CR, they should also match
the fi rst point of contact of the model to the patient's
actual fi rst point of contact, making sure that the CR
bite is accurate. You can take three CR bites and make
sure at least two out of three match. Make sure to send
them CR bites as well as protrusive bites.
Smile design is important and it's similar to how you
design dentures. Upper incisors around 10-11.5 mm.
Lower incisors around 8 mm. Length vs. width is around
8:10 ratio. About 2 mm upper incisors at repose (relaxed
lip). Thus, taking many pictures is important.
Have the lab mount the model with CR bite using
the articulator and they will wax up respecting anterior
guidance and canine guidance.
They will make sure to equilibrate, meaning only
posterior teeth are touching during maximum intercuspation
and they get separated immediately during
protrusive movement.
They will provide the stent of the model. You put that
thing in the patient's mouth with the temporary as if you
are making temporary crowns without prepping teeth.
You start prepping this temporary plus natural teeth
combo as if you are prepping virgin teeth. Oftentimes,
you prep one arch at a time.
Test-drive for about four to six weeks. Repair as
needed. Deliver the fi nal crowns.
I probably missed a few points but this is basically
what you learn in FMR classes. ■
