Needless to say, it's very hard to propose the aforementioned treatment plan to the patient
(but I understand she's satisfi ed with that). ■
Giacomo, how is a two-stage approach advantageous when it comes to soft tissue? Is it
because it's easier to do a CTG at the second stage Sx, or something else? Thanks. ■
It's not a matter of soft tissue-or, at least, not only that. In fact, it is well known that the

difference in fi nal position of the soft tissue is attributed to the underlying osseous contour and
this is true for both implants and teeth (Townsend, Olsen et al. 1985; Lindhe et al. 1987; Kaldahl
et al. 1996; Becker et al. 2001, Lindhe 2010 - textbook).
In the light of this, the distal aspect of the mesial
implant is clearly extraskeletal (horizontal bone defect),
unlike the distal implant, which has a vertical defect
with a bone peak (see the picture below).
That's why the mesial one has a soft tissue dehiscence
whereas the distal one doesn't have it.
Moral of this story: If you don't change the relationship
between implant's platform and bone crest, you won't
solve the problem with either one or a thousand CTG.
