Seth, my man, I chopped partials six years ago and never looked back. No partials, no
dentures, not even fl ippers. None of that. That's what denturists are for (or for the dentists who
love dentures). Consequently, I developed a good relationship with my local denturist, and I get
tons of implants from them.
Cheers to getting rid of stressful procedures. ■

You should be charging six to eight times your lab fee. You should use a lab that offers a free
remake if it breaks in a short period of time. Immediate dentures are a very profi table part of my
offi ce. I charge $1,500 for the immediate. In four months, I do a free hard reline. I recommend
they get a new denture after two years and I charge $1,500 for the second denture.
In your case, I would put the broken denture back together. (I personally cut keys into the
denture and use fl owable composite to hold it together.) Then take a PVS wash impression in
the denture and a bite.
Have the lab repair and reline at the same time. I would charge the patient a nominal fee.
The lab fee will probably be around $100 to $150, so I would charge $350 to $700, depending
on the patient and if they are rude or nice. ■

I guess I am just different. Maybe I say different stuff. Maybe just I am OON and charge more.
Immediate dentures can be great. You can gain so much info from existing dentition. Unless
things are really jacked up, you already got VDO and point A, size and shape. Let the patient pick
the shade they want.
If things are too jacked up, like no point A, no decent VDO, why do immediates? Just slick them
and start in a month or two. If they say they need teeth because they are in public, just tell them
OK, maybe wear a mask. If you cannot make a decent denture with VDO, point A, size and shape
of teeth, what else do you want? Bases can be relined. I charge more for immediates because I know
I will do more work. Soft liners as needed, even hard liners because why not, like it costs me more
in time or materials. I do not charge for things I may do six months from now, but I set it all out in
advance: You will need A, B, C, etc., and I will charge you at that time for what we need to do.
This allows me and the patient to retain autonomy. I am kind of upset at my CPA because
they want all clients to prepay for quarterly stuff. Really? He is a good guy who sold out. For OP,
I would do an in-offi ce repair (pretty simple repair) and hard chairside reline or impress for a lab
hard reline at that time. I would charge as I thought appropriate. These guys have to learn that
you give them plastic. Ask them how many meals they think they can get from one plastic fork. ■

It took years to learn how to do dentures. It starts with a letter explaining what expectations
are. The letter is your out. I try to make it individually specifi c, but boilerplate works. I would
suggest either a Turbyfi ll or Massad course on dentures. ■

It's worth every penny to pay for a lab that knows its stuff. Immediates are extremely predictable
in my hands. Because of my lab, not because of me. Check out Root Lab in Kansas City.
They will make you look amazing. ■
