That is a design and lack of knowledge issue. There is no need for a lingual apron, so why
add one? The autonomous cleaning function of the tongue cannot access it, so why have it? ■
I occasionally use lingual aprons for two reasons.
1. Indirect retention in cases with few teeth, and the remaining teeth are questionable.
2. The apron makes for easy tooth addition to the existing RPD. ■

The concept is called saddle extern support (or whatever the German word translates into).
That means you allow a certain amount of fl ex/travel that is supported by the rest seats. Rest
seats are on the opposite side of the active clasp arm, meaning if the active clasp arm (the lingual
is supporting, not active) is on the distal, the rest seat should be on the mesio-occlusal.
This concept allows the clasps to fl ex to the same amount of PDL/tissue movement in a
dedicated direction. Because the rest seat is on the opposite side, there is no torquing of the
clasped tooth. The saddle is not connected to the clasp arms, whenever possible. This allows for
much smaller partials and only requires a lingual bar in the lower. An apron would counteract
the movement and cause the partial to rock distally. For example: ■

Thank you to everyone who replied and had suggestions!
Update: I saw the patient again today for some fi llings I diagnosed yesterday and to take
another look at the RPD.
At the suggestion of Townies, I checked the occlusion, but didn't notice any marks on the
metal framework.
I also checked the lingual frenum in function with PIP to see if that could be lifting up the
RPD. No interference noted. The patient reported the RPD felt very stable and didn't move
around when chewing on a cotton roll.
I used light-body PVS (thanks for the suggestion, " dmd3000 " ) to see if there was space
between the LA teeth and the lingual plate. I had previously called my lab, and they said they
could do a same-day acrylic reline to fi ll any space here. There was a space and we sent the RPD
to the lab for the acrylic reline.
If for some reason the patient is still having problems with the RPD even after the reline,
or if the acrylic chips out, I was told my lab can cut away most of the lingual plate, effectively
turning the plate into a bar, so that's the backup plan.
I'll update once I deliver the RPD and the patient has had some time to try it out.
I was curious about the design concept that Uwe mentioned; it seems like a better way to go.
I wasn't able to fi nd anything when I googled that concept, unfortunately. Any clues on where
I'd learn more about that?
