I would do only 30 as well. In fact, I feel that might end up needing a root canal. I would
not touch anything else as they are not even close to the DEJ. Just be the way you are. Years of
experience means nothing. You went to dental school, not your hygienist or assistant.
I work in a practice where there is an associate who works on a different day. She collects
double the money I do in a day. Always overtreats ... occlusal fillings on all teeth even if they are
just stained grooves. Overtreats every single tooth and does not even remove the decay completely.
Really fed up of such people ... wonder how they can sleep at night. ■

30 is a gigantic lesion. That is all mush in there. 29/31 likely have interproximal. 18 and 19
have clearly visible occlusal decay. 3/4 and 13/14 I would bet cash money have interproximal
lesions when you get in there.
My Tx plan:
Start with 30, and restore 29/31 PRN once in there and I can visibly assess the interproximal
walls. Next would be 18/19 occlusal.
Now I have some clinical data to correlate to the radiographic data and decide whether to
treat or observe the other suspicious areas.
I will not comment on the HYG dictating treatment. Oops, I guess I just did. ■

I would do #30 only as well. Rest would be watch and emphasis on diet and OHI. Regular
bws to monitor. ■

I'm less concerned about the X-rays and more concerned about lack of respect from your staff.
I don't even care about the training or license aspect of being able to diagnose. But they
don't drill into these things on a daily basis for years, comparing preop X-rays to what we see was
actually there.
I don't need someone to tell me what is or isn't a cavity. My X-rays. My practice. My experience.
Sure we get surprised both ways, but we get pretty darn good at interpreting our own X-rays.
I find their comments so naive and ignorant and I think meetings are needed ASAP,
just my opinion. ■

One more vote for only doing #30(4.6). It's criminal the amount of overtreatment that goes
on. Totally agree that a lot of those incipients will become full blown caries if OHI doesn't
improve, but you don't get to make that choice for the patient.
As much as I pride myself in doing a good job, being conservative in treatment planning and
preparation, I know that any time I place a restoration, it will fail eventually. It's pretty much the
only guarantee there is in dentistry. ■
