At the close of Phase I, what is the next step with this case?

This is a case a good friend asked me about the other day (a very good ortho, in my opinion) and I thought I’d post it up here to get a little feedback as to what should be done now, or what could have been done differently. From the initial records I see two major problems that I’d like to address in Phase I – the anterior crossbite and the position of the upper right cuspid. Treatment plan would be to fix the crossbite with a 2x4 and disclusion and extraction of the upper Cs and an RPE for the ectopic cuspid.

Fig. 4: Current situation The Tx plan I described is exactly what was done and it’s been about 10 months, I think. Now what? Be happy the anterior crossbite was corrected and debonded. Phase I retainer and wait? Start open eruption? Extract upper right D to speed up the 4? Serial extract? Looking forward to everyone’s comments. My original recommendation to them was to debond and have the cuspid exposed/open eruption. I’ve gone back and forth in my head with this one. Thanks. n Andy
