I am aware of the stock size variety, the boil and bite variety, and the custom-made guards. I am continually frustrated with the ﬁt of the stock, and boil and bite. To borrow a line from U2, "I still haven't found what I'm looking for." While the custom-made guards with blocked-out areas are okay, they take extra appointments and extra work to ﬁt. In addition, they are not available the day braces are placed and patients often request mouth protection from day one. The stock and boil/bite variety either ﬁt or don't ﬁt. When I say this, I mean from the start, the arch form of the guard and the arch form of the mouth do not match. No matter what I do, this seems to be a problem without a solution. If the arch forms match, I still ﬁnd that the mouthguard does not ﬁt to a point where it does not drop when the patient is speaking. I know from the athletics I played that this type of mouthguard will not be worn. Does anyone have great ideas or products they have tried? I always say the best mouthguard is the one that is worn no matter the type but I cannot ﬁnd one that I feel the majority of my patients can or will wear. ■ APR 2 2013 str8wire Member Since: 03/03/04 Post: 3 of 19 All the non-custom ones are terrible or bulky. If you demand high quality that ﬁts over your braces you are going to have to have it made in a lab with something like ProForm. Next-day or same-week delivery of a custom mouthguard is perfectly acceptable-make sure you charge for it. ■ APR 2 2013 christjl Member Since: 02/07/13 Post: 4 of 19 I have tried and used Shock Doctor mouthguards. They are wonderful if they ﬁt. If they do not work well and you have opened the case, you cannot return them. I contacted the Shock Doctor rep and discussed this issue with him. He was very helpful. I have suggested they develop a series of trays they can give dentists that can be sterilized. The trays would represent the sizes they sell so the dentist could ﬁt the patient prior to opening a new case. I've been handing out those pre-formed ones by Masel or from the Schein catalog. I've been afraid to ask patients how they work. Could I make this on my Biostar? Just block out the braces? This doesn't seem like it would work on a patient missing teeth like an impacted canine or extraction cases. ■ APR 2 2013 Dr-T Member Since: 05/19/09 Post: 6 of 19 Regarding mouthguard design, what is the most protective design and material? It appears to me that most NBA players use a 3-4mm suck-down material for the maxilla only. My concern is that this provides only moderate protection for the maxillary anterior but no protection for the mandible or condyles. Would a mouthguard that ﬁt the upper and lower occlusion in CR provide more protection to the condyles, assuming the athlete is biting into it at the time of injury?
