Thank you for posting this case. For the upper distalization, it appears you program the
ClinCheck to distalize the U3's-U7's at the same time, instead of doing something like sequential
distalization? It seems like this approach would be faster than sequential distalization. Is that correct?
Once you have the posterior segment distalized, you close the spaces distal to U2-2? When
you are retracting the U2-2 segment, do you have your patient continue to wear the Class II
elastics full-time, at nights, or discontinue them?
I'm attempting to learn because I have a case that I'm currently working up that could use
your HSD treatment philosophy for Class II correction. My case is an 18-year-old female. ■

Yes-this case which is very severe and has other issues like crossbite still takes over two years
for me. I posted this case since it is unilateral and easy to assess the tooth movement since the
left side started and stayed Class I.

Class II elastics full-time, at nights, or discontinue them? Full-time then after retraction PM only.
Then disco about two months before retainers overcorrect a bit if possible.

HSD treatment philosophy for Class II correction. My case is an 18-year-old female. Try if possible
to time U8 extractions and good luck. Hard for me to talk Invisalign today! So disappointed! ■

I would give the extraction request at the six-week first check appointment. I start with Class II
elastics on day one. I evaluate cooperation and elastic wear. If good, then I give the extraction request.
I ask for 4-7 to be distalized in 12 aligners or more sometimes. I think 24 weeks of Class II elastic wear
is about right but you can vary for your own preferences. I generally use quarter inch 3.5 oz. elastics. ■
