TADs to intrude and roll the lowers out and palatal TADs to intrude and roll the uppers in. ■

These are tough because they require a very good patient. I have used various things to open
the bite while using piggyback expansion arches/Mulligan style arches and heavy elastics. I have
a younger patient in a lower expander right now. Everyone I have ever corrected was due to very
good cooperation. Make sure you tell the patient she has to be very cooperative or you can't fi x
it. I saw a young kid today who will not wear his elastics, and the bite is not getting better. Very
frustrating. Luckily it looks like the 7 is coming in normal, so I may be able to pick them up later
and make the save. If not, the chart is well documented about noncompliance.
Sometimes I try to get one tooth corrected at a time. ■

The surfboard turbos sound like a good idea. Do you typically just make them intraorally,
or prefabricate them?
Fen, would you fi x the bite plate to the maxillary incisors then? Will you translate that into
non-Damon orthodontic terms? This is a CuNiTi wire that is great for uprighting? I'm all in for
trying something that works well for this type of situation. ■

Certainly! CuNiTi broad archform. You can absolutely use regular NiTi too. I have used
and have several transfer cases in Damon, but it is definitely not my preferred bracket or wire. ■

My recommendation:
* Get into heavy wires ASAP.
* Place two palatal TADs and run elastics from the
buccal to the lingual TADs. I would likely just
prop bite open with a posterior BP on the right
side. Use the BP short term to jump the bite and
give her something to chew with at the same time.
On another note ... I can't see if left second and
third molars are in buccal crossbite as well? I lean toward
extracting left 8s and lower right 8 as UR7 missing. I would
have extractions done at oral surgeon at the time of TAD
placement for patient to go through "less anxiety appts."
