I'd go with Option 2. Shouldn't take too much time for root correction, given the size of
the tooth once you have enough space. Overcorrection of the torque will certainly help-a little
more predictable, IMO, in terms of fit. ■

Option 4 for me:
Ext U5s, LR5, LL2. OCS U2-4 and PT U4-6, LR4-6. Easy case and OJ will correct easily,
without any concern for too-tight OJ. ■

I would do U5s, L4s (as he's Class III tendency), bring the lateral into the arch. Do not
transpose the 2 and 3. When we've done that, it has been a mistake.
He can have all the extractions done at once under sedation, including all five 8s. ■

Something to consider. Upper 4s and two lower incisors. Below is a case I treated a long time
ago that was missing a lower incisor and I extracted upper 4s and another lower incisor. I don't
recall the patient's name so I don't have any other pictures, but I had the picture in a group that
I use for unusual extraction patterns. ■

issues and ease of treatment, just imagine the L2s are cong. missing. ■

+1 to Fenrisúlfr's post. Also, lower right 8 consult. Upper 5 extraction would require maximum
anchorage to retract 4s to present 5s position. ■

This is interesting. I disagree that we would want maximum anchorage here. Some anterior
movement of the upper incisors would not be a bad thing, IMO. Someone above suggested open
coil springs U2-4 on each side to distalize the 4s, which would also move the incisors labially. I
would suggest doing that on 17x25 SS to avoid excessive flaring. Possibly consider inverting the
incisor brackets (but that's not a definite recommendation), also to reduce flaring. This kind of
discussion is the best part of Orthotown! ■
