Extraction Pattern in This Case

This 28-year-old's chief complaint is his crooked teeth. He extracted UR5 recently.
He is mild Class III skeletal and dental. My Tx plan was to extract L4s but with the absence
of UR5, I think I will require extraction or UL5. Any suggestions are welcome. ■

I'd consider UL5 and L4s, resolve crowding, optimize incisor position and then burn anchorage. ■

U5 L4s is a good call in a Class III tendency case. It should turn out great, especially with
no growth to worry about! I love these cases.
P.S.: Don't let an OS talk you into removing the 8s. They look like good teeth to keep. ■

Would anyone consider not extracting on upper, re-evaluate for non-extraction, or one lower
incisor on lower and placing implant in upper 5 space? I would quiz the patient about airway,
sleep, etc. I rarely see a third molar I like. ■

I see all four of my third molars every morning and love them!
Here is my thinking. Take out bis and relieve the crowding, create more room for the 8s and
have a nice overall stable case that will require minimal future dentistry. Or, treat non-extraction,
extract 8s, place implant in UR, and fight retention for a lifetime. We can all treat it non-extraction.
I don't think that is a problem. Is it the best treatment plan? That is the real question.
That is exactly how I would present this case to the patient. I find most people like less
dentistry. I recall a study in which they determined that people who went to the dentist on a
regular basis, on average had fewer teeth! ■
