Has the prosthodontist mounted this case? I doubt the patient is actually Class III; he has a
collapsed VDO. If the posterior teeth were reconstructed, he would not be in anterior crossbite.
I am not seeing any way to upright the left molars. They need to have crowns on them first to
give you anything to work with, and even then you have so little anchorage to get the uprighting,
I honestly don't think it will happen.
Does this patient have $20 to $30K-at least-for this heroic plan? Seems like basic restorations
of the salvageable teeth and then overdenture-type partials would be a more reasonable way to go. ■

I'm not sure about what's going on with the condyles. Like Diane said, bilateral tends to
mean a variant of normal? But refer to OMFS for eval. But, orthodontically I don't see a lot that
can be done here.
I'm thinking this patient would be best served with removal of the lower molars, and upper
molars/premolars that are in bad shape. Then either RPD's, implant-supported bridges or
implant-supported partials seem like the way to go?
I'm not sure I would put braces on a patient with such a negligent past with regard to teeth
and oral hygiene. ■
