I think the referring GP will understand when you explain that the lower incisors will get
perio problem if you treat this case non-extraction. I would present this patient two options:
Option 1: Extract UR4, UL4, LL4; section the lower right bridge and use the missing LR6
space to retract LR 3, 4, 5. Since there is a furcation involvement on the LR7, I
really hate to extract another premolar on that quadrant.
Option 2: Extract a lower incisor and IPR the upper anterior teeth. The LR3 is in a Class III
relationship with the UR3. So I think it's safe to extract a lower incisor in this case.
I am leaning toward Option 2 to keep things simple and to keep the referring GP happy.
There are so many restorations in her mouth and some of the roots are short. I wouldn't want to
drag this case out to more than two years of treatment. ■

Kudos to you for doing the right thing. Don't ever worry about keeping the GP happy in
such situations. The patient's best interest transcends all else. If the GP is an ethical practitioner,
he or she will understand. If not, too bad, so sad.
I cannot even remember the number of times I've had crowns, implants, etc., removed for
ortho reasons. If the referral was made in a timely fashion, this might have been avoided. ■
