A young patient with a long list of considerations has Townies working together on a treatment plan
The patient is an 11-year-old boy with severe Class II, Div 1, deep bite and incompetent
lips, with overjet of 10mm. Cephalometric analysis shows skeletal Class II with mandibular
retrognathism, skeletal deep bite, proclined max. incisors and normal mand. inc. The crowding
is severe in the lower jaw. According to the cephalogram, he is not yet in the second stage of
cervical vertebrae maturation, since the inferior borders of the cervical bodies are not indented.
Would you start therapy now or wait, and what would be your appliance of choice? ■

After the usual dental stuff, expand, reevaluate for mandibular position. Pick your favorite
CL2 corrector after up righting lowers to upper. Elastics to finish. I like the upright position of
incisors to the face, so I don't really wish to retract them. ■
