Thank you for your responses. I do feel that I would normally start a case like this non-extraction
and then find myself a year in and needing to extract and having wasted quite some time. That's
why I wanted to make the call here right away. Her buccal segments are tight/tipped in, so she
would benefit from expansion.
I was planning on 5s on the right side. The left side was more questionable with the lower
midline and impacted UL3. Could possibly consider UL4 and LL4, too.
Thanks for posting the case, David; this was nicely treated. I have liked how well Invisalign
works with Class 3 adults. I think with mine, the almost completely blocked-out UL3 adds
another variable here that's making me contemplate extractions. ■

Thanks. Absolutely it's a pain to start a treatment plan and then have to reverse. However, in
your case, there are several factors going your way. The occlusal plane is very flat, and you have a
slightly overclosed facial form. There is no excessive incisor display or gummy smile. Perfect for
Class 3 elastics. You can rotate your molars and develop the maxilla. Your crowding should be
greatly diminished. Possibly IPR. I don't like to extract on Class 3 cases when I have a chance to
do these things. I prefer Invisalign because I can map up my treatment for better to start with.
Plus I hate emergencies. Good luck.
Also, uprighting the posterior allows you to retract and increases vertical dimension. Not an
easy case, but I think with care and no adverse continued growth, this can be handled. ■
