First Responder Non-Fatal Overdose TraumaInformed Care (TIC) Introduction

Prehospital Whole Blood: The San Antonio
Experience

PHD, MPH

DAVID A. WAMPLER, PHD, LP, FAEMS; CJ WINCKLER, MD, LP; RANDI

DONALD JENKINS, MD, FACS

Trauma-Informed Care (TIC) is an approach that assumes that an individual is more likely than not to have a history of trauma. TIC acknowledges
the role trauma may play in an individual's life. The National Association
of County and City Health Officials (NACCHO) and the Center for Disease
Control (CDC) Division of Overdose Prevention funded the development
of a TIC training for first responders that respond to non-fatal overdoses
either emergent or as a member of a Quick Response Team (QRT). Developing this TIC course ensures that responders are trained to perform
assessments with a trauma-informed approach. Viewing providers
through this lens gives them a new understanding of trauma they have
endured and how to manage it. This will make them a better practitioner.
We will explain to the participants the value of TIC as well as the EMS role
before and after the overdose in combating America's worst epidemic:
opioid overdoses.

Hidden in Plain Sight: Human
Trafficking in America
SUH HUGHART, EMT-P
Human trafficking is the fastest-growing organized crime activity in the
United States, averaging more than $30 billion per year. Research has
revealed that up to 88% of human trafficking victims accessed healthcare
while being trafficked. This means that the chances are high that as an
emergency responder or healthcare provider, you have treated a victim
of human trafficking and may not have recognized it. In this discussion,
learn how to recognize the signs of human trafficking, how to identify
a potential victim, and what you should do if you identify a victim. This
lecture will include key takeaway points compiled from various resources
including the human trafficking victim-witness coordinator for Central
Texas, a human trafficking victim, Allies Against Slavery, the International
Labour Organization, social services and emergency care providers, and
the Institute for Family Violence Studies.

It Takes a Village: Implementing ECMO in
Your Cardiac Arrest System of Care
JASON BARTOS, MD, PHD; KIM HARKINS, MPH
The idea of using extracorporeal membrane oxygenation-better known
as ECMO-to improve outcomes from cardiac arrest is no longer novel. In
fact, research demonstrates unequivocally that patients who remain in
shockable rhythms after unsuccessful shocks have a much higher chance
of surviving with ECMO than standard ACLS. In Minnesota, healthcare
and EMS systems have collaborated to incorporate ECMO into their
cardiac arrest system of care-their results showed such a remarkable
improvement in outcomes that ECMO is now part of their standard of
care. This was made possible only through close collaboration between
multiple hospital systems, EMS agencies and physicians, and an acknowledgement that what matters most is the patient. In this session, you'll
learn how the Minnesota Mobile Resuscitation Consortium is changing
the cardiac arrest paradigm and the steps you need to take to implement
ECMO for cardiac arrest victims in your community.

The objective of this discussion is to provide insight to starting a prehospital whole blood program based on the experiences in San Antonio
with their surrounding prehospital agencies, hospital, and blood bank
partners. The panel will discuss a systematic deployment of prehospital
whole blood, the clinical and supply benefits of using Low Titer O+ Whole
Blood, and building quality improvement into a blood program. The discussion will include regulatory issues, patient outcomes, lessons learned,
and best practices regarding FDA and American Association of Blood
Banks regulatory requirements. We will also describe a groundbreaking
South Texas plan to deliver whole blood to the scene of a mass casualty
incident and the design of a walking blood bank to deliver fresh whole
blood directly to patients and hospitals.
LEADERSHIP

Publicity, Partnerships, and Politics
ROBERT C. LAWRENCE, MCMI
2020 highlighted the need for all associations and trade
organizations that advocate for the EMS industry to come
together with one voice and purpose. What followed was
generally agreed as an unprecedented event-unity of
purpose across associations all pulling in the same direction for the good
of the EMS order. Rob Lawrence will describe how publicity, partnership,
and politics are necessary requirements to ensure our beloved vocation
is both noted and supported from the local, Board of Supervisor level to
the elected officials on Capitol Hill. Learn how to play your part in giving
EMS a voice!

Stroke Screening for Spanish Speakers
PAUL R. BANERJEE, DO; REMLE P. CROWE, PHD, NREMT; MARICELA
WILSON, BSN, RN
For every second a stroke goes untreated, 30,000 brain cells die. The most
important part of getting timely treatment for a stroke is to know and
understand the warning signs described by the FAST acronym. Our team
collaborated to create the world's first stroke recognition tool targeting
the Hispanic community, which faces elevated stroke risk. This presentation will demonstrate how this simple tool has been used in Polk County,
FL, and integrated with EMS stroke protocols to attack this problem.

The Basics of Burns and Beyond
CARL A. FLORES, NRP; JEFFREY E. CARTER, MD, FACS
seen in the field along with illustrations from recent events. We will
discuss the care from a prehospital perspective and the need to get these
patients to definitive care. We will discuss the importance of fluid resuscitation and basic care for the patients enroute, especially in transport
time is a challenge. Following transfer to burn center care, Dr. Carter will
discuss the specifics of care beyond the EMS transfer. We will discuss and
illustrate escharotomies, hydrotherapy, and laser therapy. We will reiterate the need for basic care, like keeping the patient warm due to volume
loss, as well as the importance of fluid resuscitation. We will then discuss
outcomes, both good as a result of definitive care and the less favorable
from a lack of definitive care. This will include illustrations and video of
actual patients. Finally, we will close with a Good, Better, Best practice
for prehospital providers and what they can do with given resources or
simple changes.
