a simple cold and COVID infections that viral testing is often
warranted, especially if there is an alteration of smell or taste.
Colds that last more than a week, a persistent fever over 100
degrees after the fourth day, or shortness of breath should be
evaluated because of concerns about pneumonia. These are
situations where physician office front door screening makes it
hard for those with a bad cough, sinus symptoms, or sore throat
to be seen. A phone consultation can provide guidance on how
best to address the problem.

Chest pain is always a concern-could it be a heart attack?
There are several clues that can help determine how to proceed.
Pain coming from the heart is often described as more "pressing"
or "heavy" rather than "sharp." It is also more generalized over
the chest. A pain that one can point to with one or two fingers
generally comes from the chest wall, not the heart. A feeling of
heavy pressure in the chest, especially if accompanied by sweating and/or nausea, warrants a 911 call for ambulance transfer to
the ER. The same is true if there is sudden onset of significant
shortness of breath. Otherwise, a discussion with your doctor can
determine the need to be seen urgently, with a later appointment
in the office or virtually.

Diarrhea, vomiting, and abdominal cramps can be quite
uncomfortable but fortunately usually resolve without medical
intervention. They are typically caused by a virus or something
you ate. Over-the-counter Imodium can help relieve diarrhea and
cramps. Restricting food intake, while ensuring adequate hydration by sipping tea, ginger ale, or plain water, can help until the
nausea passes. But symptoms lasting more than 48 hours warrant
a consultation. For belly pain, a simple test is to press gently on
the abdomen, pushing down about two to three inches, and then
to release suddenly. Significant pain on pressing, or especially on
release, calls for urgent evaluation.

Bladder infections are all too frequent for some women. In
pre-COVID times, a visit to check the urine was typical, but now
many providers are willing to reduce office visits by treating over
the phone without the benefit of a urinalysis. Typical symptoms
are burning with urination, frequency, and a sense of urgency.
Vaginal burning or itching suggests it may not be a bladder infection and may require an exam. Blood in the urine, if accompanied
by symptoms of infection, should clear with the treatment. If it
doesn't, or if a man has bloody urine, it should be evaluated with
an in-person visit.
