upon their return from Europe. There were 35,000 active patients in
the medical practice, which included hundreds of veterans. At that
time, there were still a few WW I veterans and many WW II, Korea
War, and Vietnam War veterans who were patients in our practice.
Many of these veterans were receiving care not only from local
community primary care, specialty care, and hospital care providers,
but also from various Veterans Administration health system providers.
It is important to determine how the patient prefers to manage
their care between various providers and health systems. Typically, it's
preferable to designate one practice or health system as the primary
health care provider and obtain most of the care at that institution.
Effective communications between provider organizations are
essential to achieving optimal care outcomes. It is important to
involve the veteran in the communication process and maintain
effective communications between multiple providers.
Communication issues can be complex between the VA health
system and community-based health systems. These challenges
are probably not much different than they are between a large
health system in Lancaster County and large health systems in
neighboring counties.
Prior to the development of electronic medical records, it was
difficult to keep up to date and medically informed on medical care
delivered outside an individual practice unless it was through mail
correspondence or a phone call. At times, a significant source of
information about a patient's care outside our office came from the
patients themselves. It is important to not only regularly reconcile
medications and lab results, but also to update visits and recommendations
with outside providers that a veteran may be seeing.
I routinely would ask the veteran to bring in a copy of a recent
community visit so that I could review it and then scan it into their
VA medical record for future reference.
Most ambulatory visits contain a medical record document
known as an After-Visit Summary (AVS), which provides valuable
information not only for the patient but also for other providers. The
AVS includes the nature of the visit, medical orders, consultation or
referral requests, current medication list, test results, and instructions
for follow up.
The veteran receives a copy of the AVS at the end of the visit
and is encouraged to share the document with his/her community
providers to better coordinate care.
If the care is coordinated and communications are effective
between various providers in multiple locations, the veteran is often
very satisfied with the flexible approach to this type of care. In the
case of veterans who may have service-connected conditions that
are followed by the VA health system, it is a high priority to ensure
continuity of care, timely communications, and the flexibility to
help the veteran optimize the care that he or she wishes to receive.
