We asked each of those systems to explain
how they go about collecting data and assessing
community need, what they have identified as
the most pressing needs, and how those needs
might be addressed.

ALICE YODER
General Health was assessing
community need long before
CHNAs were mandated,
according to Alice Yoder, Lancaster General
Health's longtime executive director of community
health.
" What we learn over time is that needs
change and must be addressed, " she said.
During the 1990s, LG Health focused on
the incidence of heart disease, cancer, and
stroke within the greater Lancaster County
community and addressed the issue of tobacco
use - a major factor in those diseases -
by implementing evidence-based tobacco
awareness programs in area schools. In 2019,
it became clear that substance abuse disorder
was a serious issue that needed to be addressed,
and the health care system responded by helping
to coordinate Lancaster Joining Forces, a
community partnership aimed at reducing the
use of opioids and heroin.
" We knew we needed to take a deeper dive
into this problem, " Yoder said. " We needed to
take a hard look at opioid disorder and figure
out what we, as a community, might do to
address this issue. "
Yoder explained that online resources such
as surveys and reports have made it easier to
track trends and engage community partners
and the general public in gathering information.
LG Health relies on surveys conducted
on social media, focus groups, and ongoing
engagement with community members
and leaders of community organizations to
provide information that is used to identify
where need is greatest. Community partners
include nonprofits, civic organizations, county
and municipal government, law enforcement
agencies, faith-based communities, and others.
* Supporting active living, healthy eating,
and less obesity
* Advocating for improvement to the county's
physical environment, emphasizing improved air
and water quality
Those initiatives are being addressed in a variety
of ways, including a recently implemented
Lead-Free Families initiative, expanding the
Food Farmacy program, and others. During
the pandemic, the hospital system has partnered
with community organizations to set up testing,
contact tracing, and vaccination sites.

UPMC is particularly looking to partner with
other systems and community organizations to
address the issues of mental health and access
to services. Diversity, equity, and inclusion has
been a longtime goal of UPMC, Nixon said, and
staff has ramped up its efforts to assure access to
services since the beginning of the pandemic,
when disparities became painfully obvious.
" We quickly realized we would need to change
the way we did things, " Nixon shared. " Our goal
has always been to meet people where they are
and take the extra steps to make sure they have

LG Health then looks at what services are
already in place to address community need, and
how those services might be expanded. Programs
may be an answer to addressing community
need, but solving a problem often relies on policy,
system, or environmental change, Yoder said.
In its 2019 CHNA LG Health identified its
four priority areas as:
* Establishing and maintaining basic conditions
that support health, including access to care,
family-sustaining incomes, accessible transportations,
affordable and quality housing, violence
reduction, and reduction in exposure to adverse
childhood experiences
* Supporting improved mental health, including
reducing and treating substance abuse
Once a community need has been identified
based on data collected it must be prioritized,
a task achieved using four steps.
The first step, Yoder outlined, is to consider the
scope of the problem, considering whether it's a
blip in time or an emerging trend, and examining
how the extent of the problem compares with
what is found in other counties and on a state
and national level.
The second step is to examine the severity
of the problem, asking whether it is negatively
impacting quality of life or resulting in lost life
years. The third step is to ask whether an issue
unjustly affects one group of people more than
others. The fourth is to examine where, out of all
the needs identified, LG Health has the capacity
to create the most positive change.
" It's our journey upstream to address root
causes of poor health. We keep working at it
in every way we can, " Yoder said. " Our mission
is to improve the health and well-being
of the communities we serve. "

UPMC Central PA
With seven acute care
hospitals and more than
160 outpatient clinics in 11
counties in Central Pennsylvania,
Tina Nixon, vice
president, mission effectiveness,
diversity equity and inclusion for
UPMC central Pa. region, depends heavily
on community partners to help assess community
needs.
In Lancaster County, which UPMC counts as
part of its Central Pennsylvania presence, Nixon
works with other health systems and community
organizations to determine how UPMC can best
serve community members.
" There are a lot of services already in place in
this area, " Nixon said. " We're not trying to recreate
the wheel or duplicate services. We're just trying
to identify needs and priorities. "
She credited other health systems that serve
Lancaster County - particularly Penn Medicine
Lancaster General Health, which has had
a presence in Lancaster for more than 125 years
- for their grasp on community health needs
and ongoing efforts to expand services.
" Lancaster General is huge in Lancaster
County, " Nixon said. " We just ask how we can
support and fill in the gaps. "
