To Philadelphia

Today, 22 cities globally participate in Cities Changing Diabetes,
ranging from Shanghai and Beijing in China to Milan and Rome in
Italy to Johannesburg in Africa and to Houston in the United States.
Houston has been the lone U.S. Cities Changing Diabetes city to
date. But that is changing this year. Novo Nordisk is bringing Cities
Changing Diabetes to Philadelphia, seeking to engage with health care
providers, health systems, health plans, faith and community-based
organizations and other stakeholders across the city to collaboratively
develop better diabetes prevention and management solutions.

The Philadelphia initiative will be guided by an Advisory Committee of leading health professional organizations, health systems,
public health, and community-based organizations, employers, and
academia.

Cities Changing Diabetes objectives in 2019 for Philadelphia
include:
* Conduct quantitative and qualitative research to understand
community-level vulnerability to diabetes in Philadelphia. This
research will use the 500 Cities data jointly developed by the
Robert Wood Johnson Foundation and CDC. In Houston the
research uncovered vulnerabilities such as feeling disconnected
from the community; not trusting the formal health care system; and suffering from time poverty that prevents individuals
from prioritizing their own personal health. To learn more

* Host a series of roundtables in early fall with a variety of
stakeholders from across the city to share the research and learn
more about needs and gaps around diabetes prevention and care
* Build a strong core team of stakeholders in Philadelphia to
help guide the overall process
* Formally launch the program in the city in Q4 with a townhall type event where Cities Changing Diabetes will share the
research and convene a panel
