Encouraging Community Action through Social Media

The community has a major role to play when it comes to
improving women's health about chronic-disease-related
factors. Community groups have formed a major part
of this involvement. Today, social media is a powerful
tool for empowering communities. Social media can be
used to encourage effective public participation, which
enables health promotion to support women finding their
own ways of managing chronic-disease-related factors.
Healthcare and other professionals can play a role in
mobilizing communities to promote women's health,
because they know how these chronic-disease-related
factors come about. Through social media, communities
can be mobilized to create healthy environments, and
various forms of advocacy. Through social media,
healthcare professionals can take up advocacy roles
working with online community groups to identify and
remove risk factors leading to an increased prevalence of
chronic-disease-related factors.

Conclusion

Chronic-disease-related factors continue to add on to
the burden of chronic diseases. Healthcare organizations
should thus find quick and efficient ways to engage the
public in seeking solutions and implementing proposed
interventions of dealing with the problem. The Chronic
Care Model is a strong framework that positions healthcare
providers to effectively deal with women's unique needs and
concerns about chronic-disease-related fatalities.

CCM exerts efforts in multiple areas allowing care
providers to focus on the improvement of functional and
clinical outcomes of women.
While there has been a remarkable improvement with
the utilization of CCM, it does not reflect the current
digitally oriented community and how the digital aspect
can provide a breakthrough beyond clinical preventive
services. Ineffective utilization of the resources that the
digital community provides makes the current CCM
unable to know the exact sentiments of the current women
of reproductive age who are highly influenced by the
digital world. The internet has many people teaching about
women's reproductive health. Not everyone is sending
out the right information and particularly, not many are
talking about chronic-disease-related factors. The fields of
health promotion and healthcare can work together with
the current CCM to make good use of the digital resources
offered by the digital community. The proposed Expanded
CCM newly includes Opinion Sharing on Public Health
