L

et's say you're a case manager on an interdisciplinary care team that
manages care for a chronically ill patient living at home.
With a packed schedule of calls and home visits (thanks to a heavy
caseload), you rarely cross paths with the home health aide who visits
the patient every other week or the primary care physician the patient
sees each month. Instead, you rely on occasional conference calls with them to
catch up, often covering multiple cases at once. Details about any one patient may
