Take Note NHS AND NJHS LOGO USAGE All requests to use the NHS or NJHS logos or names on products (i.e., apparel, cakes, etc.) must be submitted via our online usage submission form. We have discontinued the hardcopy form. This new submission process eliminates the need for faxing or mailing forms and takes less than a minute to complete. Please allow up to five business days for approval, though most requests are approved within the same business day if submitted by 3:00 p.m. (ET).
